Travel vaccines for Sri Lanka at a glance
Sri Lanka is a popular South Asia destination but the vaccine picture is more nuanced than people often expect. NaTHNaC recommends Hepatitis A and Typhoid for almost everyone, with Japanese Encephalitis added for rural and monsoon-season travel and Rabies recommended given Sri Lanka has free-roaming dogs and macaque monkeys around temples. Hepatitis A and Typhoid
Both are standard for Sri Lanka and can be given as separate injections or together as Viatim. Hep A protects for around 12 months after one dose and around 25 years with a booster. Typhoid covers you for roughly three years. These matter whether you are eating hoppers in Colombo, kottu in Kandy or rice and curry on the southern beaches.
Japanese Encephalitis and the monsoon
Japanese Encephalitis is a mosquito-borne risk in rural Sri Lanka, particularly around rice paddies during and after the monsoons. Sri Lanka has two monsoon seasons that affect different parts of the country at different times: the southwest monsoon (May to September) hits the Cultural Triangle and hill country, while the northeast monsoon (October to January) affects the east coast and Jaffna. JE vaccine is recommended for longer rural stays and monsoon-season travel. Two doses on days 0 and 28, ideally finished a week before travel.
Rabies for temples, hill country and beaches
Sri Lanka is a rabies-endemic country with free-roaming dogs and macaque monkeys around almost every cultural site, including Sigiriya, Dambulla, Polonnaruwa, the Temple of the Tooth in Kandy and many hill-country waystops. The vaccine is strongly recommended for most travellers, especially anyone staying a week or more, families with children, scooter riders and anyone doing rural cycling tours. The pre-exposure course is two doses on days 0 and 7. It does not remove the need for medical care after a bite, but it dramatically simplifies the treatment pathway and means you can wait until you are home for follow-up if needed.
Cultural Triangle, hill country and southern beaches
Most itineraries split into three: the Cultural Triangle (Sigiriya, Polonnaruwa, Anuradhapura, Dambulla), the hill country (Kandy, Ella, Nuwara Eliya, the tea train), and the southern beaches (Mirissa, Galle, Tangalle, Yala). The vaccine pack is the same across all three, but timing matters: monsoon-season trips to the Cultural Triangle or hill country need a stronger conversation about JE and bite avoidance than dry-season beach trips.
Dengue, food and water
Dengue is widespread across Sri Lanka year-round and risk rises during and after the monsoons. There is no chemoprophylaxis, so bite avoidance is everything: DEET-based repellent, long sleeves at dusk and dawn, and air-conditioned or well-screened accommodation. The pharmacist will also run through food and water rules and can dispense oral rehydration sachets and a stand-by antibiotic if your itinerary is more adventurous.
