Trekking Nepal? Here is what you need.
Nepal is a bucket-list destination for British travellers: Kathmandu and the Boudhanath stupa, the Annapurna Circuit, Everest Base Camp, jungle safaris in Chitwan, and Pokhara as a launch pad for the high Himalaya. It also has a specific travel-health profile that needs more planning than a typical beach holiday.
NaTHNaC recommends Hepatitis A and Typhoid for almost every traveller, Rabies for trekkers and longer stays, and Japanese Encephalitis for anyone heading into rural Terai lowlands during the monsoon transmission season (roughly June to October). Cholera vaccination is occasionally considered for extended rural stays. On top of vaccines, altitude planning is critical: most trekking deaths in Nepal are linked to acute mountain sickness, and a short conversation about Diamox, ascent profile and emergency evacuation is part of every Aqua consultation.

Hepatitis A + Typhoid
Standard for all Nepal travel. Available as separate vaccines or combined Viatim (one injection). Protects against food and water-borne illness in Kathmandu, on the trail and in tea houses.
Japanese Encephalitis
Recommended if you are visiting the Terai (Chitwan, Bardia, Lumbini) during the monsoon, or doing a longer rural stay. Two-dose schedule on days 0 and 28 (rapid schedule available).
Rabies
Strongly recommended for trekkers, anyone planning to be more than 24 hours from medical care, and all children. Two-dose schedule on days 0 and 7.
Altitude advice and Diamox
Pharmacist-issued acetazolamide (Diamox) prescription where appropriate, plus a written ascent and acclimatisation plan tailored to your trek.

Travel vaccines for Nepal at a glance
Nepal is not a Yellow Fever risk country, but it sits in a region where Hepatitis A, Typhoid, Rabies and Japanese Encephalitis are all relevant. NaTHNaC recommends Hep A and Typhoid for almost every traveller, with JE for rural Terai lowlands during the monsoon and Rabies for trekkers and longer stays. Hepatitis A and Typhoid for Nepal
Standard for Kathmandu, Pokhara, the trekking trails and the Terai. Both can be given together as Viatim (one injection) or separately. Hep A protects for around 12 months from a single dose (25 years with a booster); Typhoid protects for around 3 years. Worth doing even for a one-week city break.
Japanese Encephalitis
JE is mosquito-borne and concentrated in the Terai lowlands (Chitwan, Bardia, Lumbini) during the June to October transmission season. Two-dose schedule on days 0 and 28, with a rapid 0 and 7 schedule available if your trip is sooner. If you are only visiting Kathmandu and the high mountains in winter, JE is usually not required.
Rabies for trekkers
Rabies is endemic in Nepal and post-exposure rabies immunoglobulin can be very difficult to source outside Kathmandu. NaTHNaC recommends pre-exposure rabies vaccination for trekkers, rural travellers, longer stays and all children. Two doses on days 0 and 7. If you are bitten or scratched after vaccination, you still need urgent medical care, but you do not need immunoglobulin and the post-exposure course is much shorter.
Altitude sickness and Diamox
Most trekking incidents in Nepal are linked to acute mountain sickness (AMS), not to vaccine-preventable disease. Trekkers heading above 2,500m on the Annapurna Circuit, Everest Base Camp, the Three Passes or Manaslu should plan a conservative ascent profile, build in acclimatisation days, and consider acetazolamide (Diamox) prophylaxis. Aqua can prescribe Diamox at the consultation and walk you through the symptoms of AMS, HACE and HAPE, and when to descend.
Food, water and bite avoidance
Tea-house hygiene is variable above 3,000m. Boiled or treated water, hot freshly cooked food and avoiding salads or peeled fruit reduce most gastrointestinal risk. In the Terai during monsoon, DEET-based repellent, long sleeves at dawn and dusk, and an impregnated mosquito net protect against Japanese Encephalitis, dengue and (in some areas) malaria.
