Most visits combine Paro and Thimphu with a short hike at altitude, which brings two distinct health considerations: standard travel vaccines and altitude sickness.
NaTHNaC recommends Hepatitis A, Typhoid and Rabies for most Bhutan itineraries, with Japanese Encephalitis considered for rural travel and longer stays in lower lying southern districts. Malaria is restricted to the very southern lowlands bordering India and is not a concern for the standard Paro and Thimphu circuit. Altitude advice is the more important conversation for most travellers.

Travel vaccines for Bhutan at a glance
Bhutan is not a Yellow Fever risk country, but a YF certificate is required if you are arriving from a YF risk country. NaTHNaC recommends Hepatitis A, Typhoid and Rabies for most travellers. The bigger conversation for the standard Paro and Thimphu circuit is altitude.
Hepatitis A and Typhoid
Both are standard for Bhutan travel and can be given together as Viatim (one injection) or separately. Hep A protects for around 12 months from a single dose (25 years with a booster); Typhoid protects for around 3 years. Worth having for any traveller eating local food at homestays or along the trekking circuits.
Rabies
Recommended for Bhutan. Stray dogs gather around dzongs and villages, monkeys are present in the lower valleys, and rabies post exposure treatment (human rabies immunoglobulin) is very difficult to source in Bhutan. Pre exposure vaccination converts an emergency into a routine two dose top up. Two doses on days 0 and 7.
Japanese Encephalitis (selective)
JE is a risk in the southern lowland districts (Phuentsholing, Gelephu, Samdrup Jongkhar) and during rural stays of a month or more. Most Tiger's Nest and Thimphu visitors do not need it. The pharmacist will check your itinerary.
Altitude sickness for Tiger's Nest and Thimphu
Paro Taktsang (Tiger's Nest) sits at 3,120m and Thimphu at 2,334m. Most travellers fly directly into Paro at 2,235m, which is enough altitude to cause acute mountain sickness in some people. The consultation covers acclimatisation strategy, symptoms to watch for, and whether acetazolamide (Diamox) is appropriate. For higher treks (Druk Path, Jomolhari), prophylaxis is usually sensible.
