Pakistan is one of the most common visiting friends and relatives (VFR) destinations for North London families. Trips often involve extended stays with relatives in Karachi, Lahore, Islamabad, Peshawar, Multan or rural villages in Punjab, Sindh and Khyber Pakhtunkhwa, with home-cooked food, weddings and travel to multiple regions. VFR travellers carry a higher risk of food and waterborne illness than tourist travellers because they eat and live like locals.
NaTHNaC recommends Hepatitis A, Typhoid and Rabies for most Pakistan travel, with Hepatitis B for longer stays, family visits and anyone considering medical or dental care. Pakistan is also one of only two remaining wild poliovirus countries, so a polio booster (combined Td/IPV) is recommended for all travellers, and a documented dose given within 12 months and 4 weeks before departure is required by some destinations on re-entry from Pakistan.

Rabies (pre-exposure)
Recommended for longer stays, rural travel, family visits and children. Pakistan has high rabies risk in dogs and cats. Two doses on days 0 and 7.
Polio booster (Td/IPV) with documentation
Recommended for all travellers because Pakistan is a wild poliovirus country. Documented within 12 months and at least 4 weeks before departure for re-entry requirements set by some destinations.

Travel vaccines for Pakistan, at a glance
Pakistan combines two patterns that change the vaccine list. First, it is a high-volume VFR destination, so most travellers eat and live like locals. Second, it is one of only two remaining wild poliovirus countries. NaTHNaC recommends Hepatitis A, Typhoid and Rabies for most travel, with Hepatitis B for longer or VFR trips, and a documented polio booster via combined Td/IPV for all travellers.
Polio re-entry, what to know
The World Health Organization classes Pakistan as one of only two remaining countries with wild poliovirus transmission. Travellers staying more than 4 weeks may be asked, on re-entry to other countries, to show documented proof of a polio dose received between 4 weeks and 12 months before departure from Pakistan. Aqua records this on the day, signed and dated, so you have the paper trail if your onward destination asks for it.
Hepatitis A and Typhoid
VFR family meals at home, weddings, street food and rural water sources all increase the risk of food and waterborne illness. Hepatitis A protects for around 12 months from a single dose (25 years after a booster). Typhoid (Typhim Vi®) protects for around 3 years. Combined as Viatim® in one injection.
Hepatitis B
NaTHNaC recommends Hep B for longer Pakistan stays, family visits and anyone considering medical or dental care. Also relevant for piercings, tattoos and contact sports. Combined Twinrix® (Hep A + B) saves injections.
Rabies
Pakistan has significant rabies risk in dogs and cats, with limited access to modern post-exposure care in many regions. Pre-exposure vaccination (two doses on days 0 and 7) makes post-bite care much simpler, especially in rural Punjab, Sindh and Khyber Pakhtunkhwa. Strongly recommended for children and longer stays.
Malaria
Malaria is present across most of Pakistan below 2,000m. Risk is highest in Sindh, southern Punjab and Khyber Pakhtunkhwa, lower in cities like Islamabad and Karachi but still present. The pharmacist will check your itinerary against UKHSA maps and prescribe atovaquone/proguanil, doxycycline or mefloquine where appropriate, plus written bite-avoidance advice.
When to come in
Ideally 4 to 6 weeks before departure, especially if you want the Rabies course. Even 1 to 2 weeks before is worthwhile, single-dose Hep A, Typhoid and the polio booster give meaningful protection quickly.
