Costa Rica is one of Central America's most popular long-haul trips for London families and couples: Arenal volcano hot springs, Manuel Antonio National Park, Tamarindo and Nosara surf beaches, plus the Monteverde cloud forest. It is a generally low-risk destination, but NaTHNaC still recommends Hepatitis A and Typhoid for almost everyone, with Hepatitis B and Rabies for adventure or longer trips.
Yellow Fever is not a risk inside Costa Rica itself, but the country requires a Yellow Fever certificate if you are arriving from a YF risk country (much of sub-Saharan Africa and tropical South America), including layovers of more than 12 hours. If you are combining Costa Rica with Panama, Colombia, Peru or Brazil, plan the YF vaccine and certificate accordingly.

Travel vaccines for Costa Rica, at a glance

Hepatitis A and Typhoid
Both are standard for Costa Rica and can be given together as Viatim (one injection) or separately. Hep A protects for around 12 months from a single dose, and 25 years with a booster. Typhoid protects for around 3 years. These cover the food and water risks across Arenal, Monteverde, Manuel Antonio and the Nicoya peninsula.
Hepatitis B
Recommended if you are staying longer than a month, doing adventure activities (surfing, ziplining, white-water rafting), volunteering, or anticipate any medical or dental care during your trip. A standard three-dose schedule or accelerated course can be tailored at the consultation.
Rabies
Worth considering for cloud forest treks, longer rural stays, and anyone planning to be more than 24 hours from quality medical care. Two pre-exposure doses on days 0 and 7. Highly recommended for children, who are more likely to interact with stray animals.
Yellow Fever (transiting travellers only)
Costa Rica itself is not a Yellow Fever country, but it requires a valid ICVP if you are flying in from a YF risk country with a layover longer than 12 hours. Common triggers: connecting via Panama City from Bogota, Lima, Quito or Sao Paulo. If your routing touches a risk country, get vaccinated at least 10 days before arrival in Costa Rica. Aqua signs and stamps the certificate in-clinic.
Mosquito-borne illness and bite avoidance
Costa Rica has Dengue, Zika and Chikungunya year-round, particularly in lowland and coastal areas like Tamarindo, Manuel Antonio and Puerto Viejo. There is no vaccine for these in standard UK practice, so prevention is everything: DEET 50 percent during the day, long sleeves at dawn and dusk, and air-conditioned or screened accommodation where possible. Malaria risk is very low and antimalarials are not routinely recommended.
