Who should consider the Typhoid vaccine
NaTHNaC recommends typhoid vaccination for travellers visiting areas of moderate or high typhoid risk, particularly those who will stay with friends and family, stay in basic accommodation, eat from street stalls, travel for more than a few weeks, or visit rural areas. Common destinations include India, Pakistan, Bangladesh, Nepal, Sri Lanka, Egypt, Indonesia and Ethiopia. Vaccination is especially valuable for VFR travellers (visiting friends and relatives), who carry the highest typhoid risk in UK epidemiology.
Schedule and doses
Typhim Vi is a single 0.5 mL intramuscular injection giving protection from around 14 days post-dose for approximately three years. Vivotif is an oral course of three capsules taken on days 1, 3 and 5, finished at least seven days before exposure. A combined Hepatitis A and typhoid product, Viatim, is useful when both vaccines are needed in one visit. The pharmacist will recommend the option that fits your itinerary, age and medical history best.
Common destinations and pairings
Typhoid is almost always paired with Hepatitis A, because the two share the same food and water route. For longer rural stays, Rabies may be added, and a booster of Tetanus, Diphtheria and Polio is often due. Side effects and contraindications
Typhim Vi commonly causes a sore arm, mild headache or low-grade fever for one to two days. Vivotif may cause mild nausea or abdominal discomfort. Vivotif is a live oral vaccine and is not suitable for people who are pregnant, severely immunosuppressed, taking antibiotics, or for children under six. Typhim Vi can be given to children from age two, and is the usual choice in pregnancy where vaccination is needed. The pharmacist will screen for all of this during the consultation.
